Like all Uniques in Diablo 4, there is no guaranteed drop for the Tempest Roar, meaning you have to rely solely on RNG and luck. However, there are ways to boost your odds through target farming.

The best way to farm for Tempest Roar in Diablo 4 is to defeat the Echo of Duriel or the Echo of Andariel, both of which have the Tempest Roar in their loot pool with an increased chance of receiving the item.

Fighting those bosses requires summoning materials, however, but the good news is that Tempest Roar can drop in other activities like Gathering Legions, World Bosses, Helltides, and Nightmare Dungeons—with activities in increased difficulties having better chances.

Utilizing those other methods will come in handy when hunting for Tempest Roar, as both Duriel and Andariel are troublesome to summon as they require materials earned by defeating other bosses—Echo of Varshan and Grigoire the Galvanic Saint for Duriel and Lord Zir and The Beast In The Ice for Andariel.

If going down the Duriel route, complete Tree of Whispers rewards or kill Treasure Goblins to earn Malignant Hearts, used to summon Varshan, while also focusing on opening Tortured Chests during Helltide events to earn Living Steel, the summoning material for Grigoire. Once you have the necessary materials to summon Duriel, defeat him and repeat the process again until you get the Tempest Roar.

Alternatively, you can head down the Andariel route. The first step is to earn Exquisite Blood by defeating the Blood Maiden in Helltide, completing Legion and World Boss Events, and opening Whisper of the Dead caches, then using the Exquisite Blood to summon Lord Zir.

The other part of the Andariel route is to focus on Nightmare Dungeons for Distilled Fear, used to summon the Beast in the Ice. Once you’ve got the necessary materials from both Lord Zir and the Beast in the Ice, summon Anderial, defeat him, and repeat the process.
