A Diseased Heart, on the other hand, is one of the more useful pick-ups as you continue your journey, since it can mean the different between life and death.

What is a Diseased Heart in Cult of the Lamb?
A Diseased Heart is a form of health that works similarly to Blue Hearts, where they only last until the end of that specific crusade. Unlike Red Hearts, Diseased Hearts and Blue Hearts will be lost forever, and cannot be regenerated through normal means, such as a Heart Container. As a result, they are much more valuable to hold as you run through a dungeon.
The difference between a Diseased Heart and a Blue Heart, however, is that when a player takes a hit and loses a heart, they will deal damage to all enemies on the screen. If you’ve collected all three types of Hearts, the Diseased Heart will be expended first, followed by the Blue Hearts, then your base Red Hearts.
How to get Diseased Hearts in Cult of the Lamb
You can only get Diseased Hearts while in a crusade, and must be acquired from random chests or through a Tarot Card effect. You can also get Diseased Hearts by equipping the Fleece of the Diseased Heart, which gives you a Diseased Heart every time you draw a Tarot Card. You can also find Diseased Hearts hidden among breakable items, so don’t be afraid to destroy everything that you can within each room that you visit during a crusade.
Is it worth getting Diseased Hearts in Cult of the Lamb?
Since Diseased Hearts help with durability and room clearing efforts, they are very important to have while on crusades and are worth acquiring if you have the chance. In the early stages, these hearts can help out with clearing tough rooms, while still keeping you healthy for that specific raid. They are, however, usually random drops, so you should pick up as many types of hearts as you can, no matter what type.
